Job summary

This exciting opportunity will enable the post holder to work with the Liverpool Community Learning Disabilities team. You will work among a range of professionals including Community nurses, Health Facilitators Speech and Language Therapy, Physiotherapy, Psychology, Psychiatry, and Intensive support service.

The new create post - The Enhanced care Home Learning Disability Practitioner will be a key contributor to ensuring the quality and access of people with learning disabilities to primary, secondary, and tertiary healthcare within the National Health Service.

Main duties of the job

To support the delivery of the Enhanced Health in care homes framework. To collaboratively work with a range of partners, including community colleagues, primary care networks and third sector, to ensure delivery of the framework and associated specifications.

Adopting and enabling a person-centered care program approach across ensures the full participation of all individuals in valued community life experiences. The post holder will work as a member of the specialist multidisciplinary team carrying out assessments and providing effective operational and clinical support to people with a learning disability who live in CQC registered care homes, ensuring those with complex needs are enabled to access healthcare effectively.

Work with local Primary Care Organisations to ensure the needs of people with a learning disability are taken into account within the Clinical Commissioning Group (CCG) to enable them to develop their strategic plans. Support the delivery of the Enhanced Health in care homes framework. To collaboratively work with a range of partners, including community colleagues, primary care networks and third sector, to ensure deliver of the framework and associated specifications.

The post holder will be expected to work proactively with CQC registered care homes to assess, plan, and where necessary advise on the delivery of holistic plans of care for the people with learning disabilities, whom they support. This may be in collaboration with other partners, such as enhanced health care home matrons or GPs.

The definition, development and delivery of valued and expert specialist health supports which promote the independence, community inclusion and positive reputation of people with learning disabilities and their carers. Assuming a lead in the continued development of effective clinical services to meet the complex needs of people with either behaviours of concern or mental health problem through direct and indirect work, training, supervision, co-working and consultation.

To provide a range of appropriate evidenced based inventions based on the assessed need of the referral and result in effective care programme that can be utilised across the service both in the community, acute and residential settings.

To undertake on occasions urgent or emergency assessments, including risk assessments.

  • Educated in relevant subject or equivalent level qualification or significant experience of working in a specialist area

  • Educated to Masters level - with Advanced Clinical Practice
