Job summary

We are looking for a qualified practitioner to join our team.

The post holder will ensure the provision of Crisis Resolution in a variety of care/domestic settings, providing leadership and supervision throughout each span of duty, ensuring standards of clinical practice and risk management is maintained.

Main duties of the job

1. To provide clinical advice, expertise and leadership within your team for Older Persons.

2. To coordinate the care of a defined group of service users in the community, carrying out their assessment and planning implementing and evaluating their care under the guidelines of ECC, without supervision.

3. To operate as the clinical focus for day-to-day planning of treatment, risk assessment and management of the quality of care within a multi-disciplinary team setting.

4. To ensure a seamless approach by liaising with other professionals as required e.g. A&E, ward-based staff, community services.

5. To ensure that the service users care plan is developed to meet the assessed need of the individual. To monitor, via performance management, the care plans developed for service users by all patch / sector members to ensure that they are appropriate and of a high standard.

6. To liaise with family members, carers and significant others when completing the assessment and developing a care plan. To be the first point of contact for service users and carers that have concerns regarding their care, making every effort to resolve their concerns sensitively and promptly, referring to and informing the CRHT Manager as necessary.

7. To develop a risk management plan for service users under your care, taking note of the contributions from other members of the team. Ensure that all significant people are aware of the risk management plan and are actioning it appropriately.
