Job summary
Come and join us at Leigh Moss Community Hub, at Leigh Moss Hospital, Liverpool L14. We are currently recruiting band 5 nurses to work within our community mental health teams.
We can offer you an exciting role that will interface with older people's functional care pathways, dementia care pathways and our young onset dementia care pathways.
There is plenty of opportunity to work alongside our psychology team, we encourage and support our nurses to deliver psychological interventions such as post diagnostic groups and cognitive stimulation therapy. We will guarantee career development through a variety training packages and learning opportunities. We have an excellent multidisciplinary team with a wealth of experience here at Leigh Moss.

That said, we welcome applications from nurses who would like to move into this specialist community service recognizing full transition training can be offered.
Main duties of the job
Provide assessments and develop treatment plans for service users on caseload Hold a caseload of people with cognitive and functional mental health problems.
Participate in team meetings , MDT meetings and training opportunities.
Provide support for service users and carers
Participate in delivery of group interventions
Liaise with colleagues and coordinate care for service users on caseload
Support and monitor the service user whilst commencing medication
Work collaboratively with other members of the MDT
Attend regular supervision meetings
