Job summary

The Community Mental Health Transformation is a three year project and is expected to radically transform the way we deliver services with an emphasis on an integrated care model.

An approved model and framework has been developed for Cheshire & Merseyside by the three NHS mental health providers and this can be seen below. The ethos of this is that service users are able to step-up/ down from services quickly and easily as needed ensuring they have access to the right person/ group in the right place at the right time, with wider communities playing the main part in providing ongoing support.

The role of the Consultant for Primary Care Mental Health will be crucial in facilitating transforming the culture of the system to ensure that mental health is seen as everyone's business and to support wider community involvement moving towards most care being provided by our voluntary and third sector partners. This will ensure that when people need to step-up for additional specialist support they receive this quickly to prevent relapse or exacerbation of symptoms.

Main duties of the job

The postholder is not expected to manage and hold a caseload, but to provide a consultative model to support MDT working with GPs and PCMHPs. The role will require excellent communication skills in order to develop good working relationships with primary care services. The role will require liaison between primary and secondary care services as part of wider MDT working. There will be an educational element offering teaching, advice and support to GPs, Pharmacists and PCMHPs

It is anticipated that the team members will have clear and open communication with each other.

The post holder will operate within a multi-disciplinary team, which includes health staff and wider communities.

The post holder will be expected to lead MDT discussion at local and Trust level maintaining close relationships with consultant colleagues in secondary mental health teams to support smooth pathways and allow an easy step up-step down approach for those people accessing services. It is expected that the postholder would be responsive and flexible to the needs of the team.

  • MB BS or equivalent medical qualification.
  • MRCPsych

Eligibility

  • Fully registered with the GMC with a licence to practise at the time of appointment.
  • Included on the GMC Specialist Register OR within six months.
  • Approved clinician status OR able to achieve within 3 months of appointment
  • Approved under S12 OR able to achieve with 3 months of appointment
