Job responsibilities

CLINICAL

Undertake personality disorder assessments and develop Living Life Well Crisis Care Plans

Case manage clients who have been clustered as 8 and who frequently present in ways in which current provision struggles to manage effectively

Facilitate admission and discharge for clients they case manage or who are on the personality disorder pathway

Formulate and implement appropriate treatment plans and provide Structured Clinical Management

To help facilitate and deliver Dialectal Behavioural Therapy / Mentalisation Based Therapy informed therapeutic interventions and/or groups to service users.

Provide short term goal focused support / input to cases on pathway

Liaise with other professionals to contribute to the establishment of continuity of care across service

Be point of contact for primary and secondary care interface

Act as point of liaison working in collaboration with Inpatient, Assessment, HTT and Acute Services

To ensure that continuity of care is maintained and to support services to manage transitions between services in line with NICE guidance including that Section 117

To become part of the Centre of Expertise in Personality Disorder and once a month meeting

To carry out assessments as per Patient Owned Database and assign patients to care clusters within the framework of the Health of the Nation Outcome Scales Payment by Results (HoNOS and PBR)

Manage meetings

To be able to provide consultation and training in personality disorder

Demonstrate an ability to formulate clients mental health needs

Agree to undertake additional training when required to facilitate role

To support the adherence of the personality disorder pathway.

The post holder may be required on occasion, as part of this role, to assist and support colleagues within the wider Team.

There is an expectation to display strong and effective communication skills in undertaking this role. You will be required to liaise with various, multi-agency professionals as part of this role, to provide and receive highly complex, highly sensitive information. You will contribute to decision making process, regarding complex case management matters. This will require you to display empathic, motivational and effective interpersonal techniques, whilst demonstrating well informed clinical judgement and leadership.

MANAGERIAL

Provide clinical supervision of staff to support current Pathways in regards Personality Disorder.

Provide clinical leadership to colleagues and wider staff within Borough, to support the development of clinical pathways.

Contribute to the development and implementation of clinical pathways within the borough.

As part of the wider Team, you will be expected to contribute to efficiency savings, with innovative or creative ideas, that may enable the Team to operate in more cost effective ways.

You will be responsible for the clinical supervision of qualified staff, in support of clinical pathways.

You will take a lead role in regards to training and further development of qualified staff, to support the development of clinical pathways.

You will contribute to the development of clinical pathways in the Borough/Trust, including the relevant policy and procedural guidance to support and inform clinical practice.

Qualifications

Essential

• Up to date Professional Registration (to be maintained)
• Evidence of continuing professional development in the field of RMN; or / Occupational Therapist with post qualification and further training in Personality Disorder.
• Completion of relevant course in DBT/MBT or a willingness to undertake DBT or MBT training.

Experience/Knowledge

• Able to demonstrate an understanding of the Personality Disorder Pathway model
• Working experience of Inpatient / Outpatient settings
• Experience of formulating risk assessments
• Experience of working with mental health services (or able to demonstrate transferable skills)
• A working knowledge of NICE Guidance protocols
• A clear understanding of complex needs support issues such as homelessness, substance misuse and offending
• Knowledge of safeguarding protocols
• Knowledge and understanding of how to deal with challenging individuals
