Job summary

Integrated Community Reablement and Assessment Service (ICRAS) - exciting opportunities to join an evolving service as a Therapy Coordinator, with an Physiotherapy background.

The vision for ICRAS is that a multidisciplinary team work together to manage the care of people both in the community, as well as those transitioning. We aim to prevent inappropriate and/or avoidable hospital admissions by providing short term, intensive, holistic assessment treatment of care.

Individuals who have a good understanding of community based therapy and are comfortable within a range of settings, flexible and able to travel independently.

Main duties of the job

The post holder will provide expertise and advice in the treatment of complex conditions and have continuing responsibility for the management of a defined caseload and a coordination role of daily activities. Responsibility for the continuity of care and be the lead person for communication to identified stakeholders. The primary function of the role is to provide proactive, responsive services that address the anticipatory and maintenance care needs of the patients/clients within the defined caseload.

Practice autonomously and demonstrate evidence based clinical decision making.Work within guidelines to identify and manage risk, reporting identified risks to the team leader. Ensure the delivery of the quality strategy within the designated area incorporating establishment of systems and processes.In addition the post holder will use their expertise to be a source of training and mentorship to those within the team and as necessary, more widely.
