Physicians Associate supporting the elderly and vulnerable population
Band 6 Hours: 37.5 week
An exciting opportunity has arisen to work in a newly created role providing an integrated approach to supporting the physical and mental health needs of those living in Care Homes (both elderly care and learning disability homes); those with Learning Disabilities; and those patients whose health conditions cause them to be housebound. This is a joint venture between Mersey Care NHS Foundation Trust and Seaforth & Litherland Primary Care Network, comprising six General Practice organisations in South Sefton (Bridge Road Medical Centre, 15 Sefton Road, Glovers Lane, Ford Medical Practice, Primary Care 24, and SSP).
Main duties of the job
The post holder will undertake physical health checks, advise on lifestyle factors, ensure access to screening and vaccination, perform physical assessment and request or interpret basic investigations, and deliver personalised care plans, working within an extensive multi-disciplinary team including (but not limited to) General Practitioners, Practice Nurses, Community Matrons, District Nurses, Community Geriatrician and Care Home staff. The role will be based between the six General Practice organisations of Seaforth & Litherland Primary Care Network, but will require flexibility to work in a variety of settings including remotely, in patients homes, supported living accommodation or care homes. The post holder will need to be an excellent communicator and will be required to use a variety of engagement modes including face to face, telephone, email and video consultations. .

- Postgraduate Diploma in Physician Associate studies including having passed the national examination for Physician Associates
- Registration on the Faculty of Physician Associates Managed Voluntary Register -post qualification

Knowledge
