Job summary
An exciting opportunity has now arisen within the Manchester Macmillan Palliative Support Service for an Assistant Practitioner. Please note the foundation degree is an essential requirement for this post.
The post holder will work within the Macmillan Specialist Palliative Care Team, contributing to the provision of a co-ordinated and comprehensive service for patients receiving specialist palliative care including patient's resident in care homes in South Manchester.
The post holder will work as part of the multi-disciplinary team comprising of Doctors, Nurses, Dietician, Occupational Therapist, Physiotherapist and Speech and Language Therapist. The team aims to provide effective palliative care andsupport to patients with any life-threatening illness.
Main duties of the job
The post holder will visit patients in their own homes or are home and assist other professionals in the implementation, delivery and evaluation of palliative care and education programmes to promote independence and encourage improved health, functional capabilities, and quality of life. They will support patients and their families as they approach the final stages of their lives.
The post holder will hold a small caseload under the supervision of the responsible clinician, supporting patients with life limiting illness to maximise their health and enable them to be cared for in their preferred place.
The post holder will need to demonstrate how they will meet the ability to be mobile within community.
