Main duties of the job

The post holder will work as an integral part of the Intermediate Care Therapy team (and the wider MCR service) to provide a high quality patient centred approach to adults in the community in North Manchester.

The teams integrated service delivery model aims to avoid admissions, reduce length of stay and improve the patients' experiences through access to the right intervention at the right time, delivered by the appropriate health or social care practitioner.

The aim of the post is to rehabilitate and improve the function of patients by providing interventions in their own home either following discharge from an acute hospital setting or following illness or injury at home.

You would be part of a wider multi disciplinary health & social care team who work towards regaining or maintaining independence and quality of life. The post holder will have an allocated caseload of patients under the supervision of a registered professional.

The role will also involve team/office based duties to ensure effective service delivery.

The team is committed to continual professional development with regular supervision and appraisals. This post is supported by B5, B6 and B7 Physiotherapists and Occupational Therapists.
