Job summary

An existing opportunity has arisen to join Central Crisis Response team. Working as part of a multidisciplinary team and working across the primary, secondary and social care in Manchester, the post holder will be responsible for supporting the Crisis Response Team to deliver urgent health care to people in crisis and at risk of hospitalisation, or who require a timely discharge and support from a more intensive level of care, such as an intermediate care environment. The Crisis Response Team will also receive direct referrals from NWAS (North West Ambulance Service) as well as from Acute and Primary Care, and Community referrals.

Main duties of the job

You will work in the Community based Crisis Response Team (part of Manchester Community Response) assisting registered professionals (Advanced Clinical Practitioners, Physiotherapists, Occupational Therapists, Nurses, Pharmacists and Social Workers). Working as part of a multidisciplinary team and working across the primary, secondary and social care in Manchester, the post holder will be responsible for supporting the Crisis Response Team to deliver urgent health care to people in crisis and at risk of hospitalisation, or who require a timely discharge and support from a more intensive level of care, such as an intermediate care environment. The Crisis Response Team will also receive direct referrals from NWAS (North West Ambulance Service) as well as from Acute and Primary Care, and Community referrals.

  • Foundation Degree in Health and Social Care

Desirable

  • NVQ level 3 in Care - Health related qualification - IT qualification or recent in-service training

Experience.
