Job summary

An exciting opportunity has arisen for a highly motivated, experienced, and enthusiastic Advanced Clinical Practitioner to join the Specialist Palliative Care Team in Trafford Local Care Organisation.

The post holder will be an autonomous practitioner who will provide clinical leadership to the Macmillan Palliative Care Team, working in collaboration with the Service Manager and Clinical Lead. They will take a lead role in the clinical management of patients with specialist palliative care needs, in a community setting. The post holder will contribute to a programme non-clinical activity including, although not limited to, quality improvement, education and audit.

The post holder will work with key stakeholders within the organisation and primary care to ensure a high standard of evidence based palliative and end of life care provision across the Trafford locality.

Main duties of the job

The post holder will be expected to take an active role in the advanced assessment, management and treatment of patients referred to the community Macmillan Service. They will provide comprehensive healthcare sensitive to the needs of the Trafford population, including advanced clinical assessments, history taking, diagnostics, treatment planning, prescribing and evaluation. They will utilise advanced communication skills to assist patients to proactively manage and anticipate their future health needs, ensuring that advance care planning is embedded.

Working as part of a multidisciplinary team within the Locality and with colleagues across Manchester, the post holder will work across primary, secondary and social care, hospice and tertiary centre boundaries and demonstrate clinical leadership when leading and coordinating the specialist nursing palliative care offer for people with highly complex needs. They will support teams to respond in a timely manner for patients potentially at risk of hospitalisation, supporting patients to be cared for and die in the place of their choice.
