Job summary

Buccleuch Lodge Intermediate Care Unit is long established in the West Didsbury area. We have a unique Community setting with dedicated nursing and therapy staff. We are a well structured, motivated team with a strong patient focus. We are a constantly developing service with nurse led initiatives. We provide inpatient care to 14 patients providing rehabilitation with the aim of returning patients home with support.We can provide career development and educational possibilities and invest constantly in our staff and service development and modernisation. Main duties of the job

Undertake comprehensive health care needs assessment of patients.

Collaborate with other health care professionals.

  • Use professional judgement when assessing patient needs/ problems and investigations requiring analysis and intervention.
  • Ensure that patient / ward information is communicated to the multidisciplinary team.
  • Practice in accordance with the NMC Code of Conduct and the Trust's policies and procedures, and professional guidelines in order that a safe and quality service is provided.
  • Facilitate and encourage patients, carers and relatives to take an active role in care given and where appropriate develop plans that enable patients and carers to be independent as far as reasonable possible.
  • Communicate effectively with dignity and respect to all patients, relatives, staff and others.
  • Provide and receive complex, sensitive information concerning patient's medical condition.
  • Undertake practice sensitive to the individualised needs of all patients,
  • Administration of medicines and treatments in line with NMC & Trust Policy
  • Promote self-care and health education in the clinical ward/department.
  • Be actively involved in all aspects of admission and discharge, engaging with other agencies and community teams to ensure a safe, individualised approach based on the assessment of needs.
  • Be competent in the correct use of all equipment used in the clinical setting.
  • Maintain accurate, contemporaneous patient records in line with Trust Policy and NMC Code of Conduct.

  • Registered Nursing Associate on the NMC Register with a Foundation Degree
  • GCSEs Grade C or above in Maths and English Language / New GCSE Grade 4-9 in Maths and English Language OR Functional Skills Level 2 in Maths and English or Equivalent (not entry level 2)

Knowledge

Essential

  • Understanding of the scope of the role of the Nursing Associate in context of the nursing and interdisciplinary team and the organisation, and how the role may contribute to service development
