Job summary

An excellent opportunity has become available for an enthusiastic and highly motivated Discharge Co-Ordinator to join our supportive Integrated Discharge Team at Manchester Royal Infirmary.

We are looking for an individual who can be part of our fast-moving, innovative and ambitious team; dedicated to supporting our patients and together with our clinicians and ward staff to help the Trust in managing our patient's journey from admission through to discharge.

The ideal candidate will be dynamic, adaptable and have the ability to identify issues, provide solutions, have a team focused attitude and demonstrate that patient care and patients safety is at the forefront of any decision making process.

Main duties of the job

  • Identification of medically fit patients
  • Reporting any delayed discharges in line with Reason to Reside guidelines.
  • Effective communication with various departments and disciplines to ensure safe and timely discharges can be met.
  • Assessment for patient's care needs on discharge.
  • Work Flexibly across a variety of settings.
  • To be competent with IT systems.
  • Establish partnerships and networks with wider service providers and agencies.
  • To work closely with the hospital discharge service in ordering and chasing equipment for home discharges.
  • Effectively communicate with patients, families, other services and wider networks.
  • Assisting patients and carers to access information, or signpost as required.
  • To have an understanding of the Social Care requirements and legislation including the Mental Health Act, Mental Capacity Act and Care Act.
