Job summary

We are Manchester Local Care Organisation - an exciting, new public sector organisation that is bringing together NHS community health servicesand social care services in the city. A Specialist Early Help Case Planning Lead position has become available within the Specialist Case Planning Team.The duties of the post are contained within the attached Job Description; the successful applicant would work within the city wide team based atRusholme Health Centre to facilitate Specialist Case Planning as part of the extended Health Visiting Service throughout Manchester.

Main duties of the job

As Specialist Case Planning Lead, the post holder will

Co-ordinate and facilitate the case planning process for children with additional needs.

Be responsible for robust, quality, outcomes focused case planning arrangements for children and their families/carers with additional needs which may result in neglect.

Act as role model and motivate colleagues in working towards an innovative and high standard of practice to improve outcomes for children with additional needs.

Act as health liaison to Early Help Hubs to provide health advice and guidance.

Ensure the timely production and circulation of accurate minutes, records of meetings and agreed actions by individuals and agencies involved in meetings.

Co-ordinate and report on Early Help activity, support the lead professional role and case planning activity within staff teams.

Provide a city wide case planning service with colleagues.
