Job summary

Manchester Local Care Organisation have an exciting opportunity for an Band 6 physiotherapist to join the Home pathway Team.

The successful candidate will be part of a highly experience and dedicated Team of physiotherapists, Occupational Therapists, nurses and Assistant Practitioners. The service is an Intermediate Care at home service providing care for patients in their own home, providing goal centred treatment for patients with a variety of conditions with the aim of optimising independence and reducing risk of unnecessary hospital admission. They will be enthusiastic and have a passion for delivering high quality services to the population of North Manchester.

The Home pathway Service currently operates Monday- Sunday, 8:00 - 20:00. The successful candidate must hold a full UK driving license and access to their own vehicle for work purposes.

Main duties of the job

To provide a high standard physiotherapy service for the Home pathway Team - intermediate care at home.

The post holder will be responsible for providing highly specialist assessment and treatment for patients within the Intermediate Care Service.

The post holder will be responsible for providing specialist assessment and treatment for patients in the community who have been discharged from the acute sector, or have been identified as needing physiotherapy input as part of an integrated, multi-professional and multi-agency Manchester Community Response.

The post holder will follow agreed protocols for ensuring appropriate and timely input. They will also ensure that pathways of care and communications are maintained with all agencies working with the service.

The post holder will work as a member of the Manchester Community Response Service, ensuring individual needs are met, enabling choice, participation in rehabilitation and an improved quality of life for the patient.

The post holder will aim to prevent re-admission to hospital or inappropriate admission to residential or nursing homes by maximising function and independence of patients in their own homes and their community, whilst respecting patient choice and autonomy.
