Job summary

An exciting opportunity has arisen to recruit a full time Discharge Coordinator as substantive Band 6 post for ward 78.Ward 78 is a 32 bedded ward, specialising in Neurosurgery, Neurology, Trauma and Orthopedics, Complex Spines and Ear Nose and Throat. We offer acute and long term care for patients. We are one of four centers that offers Epilepsy Surgery (NORCESS) and work closely with Alder Hey to offer this complex surgery.

The role of the discharge co ordinator would involve working closely with the multi disciplinary team and families to promote a safe and effective discharge from hospital. This would involve providing training such as Gastrostomy training & Tracheotomy training to families and assessing their competencies to ensure they are carrying out safe cares. The discharge co orindator would be responsible for attending discharge planning meetings and any safeguarding meetings to represent the ward.Liasing with all families on the ward on a day to day basis would be a key part of this role to sign post to services which they may require.As well as providing training to families the discharge co ordinator will also work closely with the practice educator to provide a high standard of training to staff on the ward.

Main duties of the job

The post holder is responsible for facilitating safe discharge to relevant client group and will work with the specialities on ward 78

The post holder will work as part of the wider multi-disciplinary team to promote efficient discharge. She/he may also provide direct patient care and have detailed clinical knowledge of the specialist area.

The post holder will have good knowledge of social services and safeguarding processes.

The post holder will assist the senior Sister/Charge Nurses in ensuring that high standards of quality of care are delivered & maintained. The post holder will act as a positive role model within the department for staff patients and carers.

The post holder will deputise for the senior sister as required.

The post holder will enable nurses to acquire clinical and professional competencies by facilitating high quality education and staff development opportunities. Post holders will be expected to participate fully in their personal development.
