An exciting opportunity has arisen for a highly motivated and enthusiastic band 6 Physiotherapist to join the Community Hospital at Home Team in Central Manchester.
We are an enthusiastic, motivated multi-disciplinary team working in a dynamic, exciting environment, providing a hospital avoidance/ supported discharge service.
The postholder will provide and/or arrange appropriate support and interventions in the community working closely with colleagues across all sectors. The post holder will provide specialist knowledge and clinical skills to ensure the delivery of care within the service and the wider community and across the acute / community interface. They have a fundamental role in the delivery of high standards of care, ensuring an environment that is conducive to well-being. They will actively participate in improving the service the department/unit offers and will initiate and support future developments.
