Job summary
The Associate Macmillan Palliative Care Nurse will work as a member of the wider locality based multidisciplinary Macmillan Palliative Care Supportive Service.
The post holder will support Neighbourhood teams to deliver locally driven high quality care, responsive to patient and carers' needs. This includes education of other health care professionals, training, support and advice for the multidisciplinary team.
The focus of the multidisciplinary approach will be to support patients and carers to live well, prevent inappropriate hospital admission and meet their preferred priorities for care.
Part time (30 hours and above) would be considered.
Main duties of the job
An exciting opportunity has arisen for an enthusiastic, motivated, innovative nurse to join our Macmillan Specialist Palliative care team in South Manchester.
The team is a multi-disciplinary with Palliative Medicine consultants, an Advanced Nurse Practitioner, 4 Clinical Nurse Specialists and 4 Associate Palliative care nurses. A specialist physiotherapist and Occupational therapist, dietician, SALT, and a social worker. We have a weekly MDT where we present all our new patients and discuss with our Palliative medicine consultants.
The Associate Palliative care nurse will be supported by a band 7 Clinical nurse specialist and work closely with a band 4 assistant practitioner.
The team is stable and supportive with a wealth of expertise and experience that we are keen to share. There will be opportunities for level 7 study to undertake clinical skills / non-medical prescribing modules. A full and individually tailored induction will be provided.
The post holder will provide high quality, specialist palliative care to patients in their own home. offering symptom control and psychological support. They will work autonomously whilst having access to support form senior colleagues and medics.
This post is full time, but we would be willing to consider 30 hours and above for the right candidate.
