Job summary
The Associate Macmillan Palliative Care Nurse will work as a member of the wider locality based multidisciplinary Macmillan Palliative Care Supportive Service.
The post holder will support Neighbourhood teams to deliver locally driven high quality care, responsive to patient and carers' needs. This includes education of other health care professionals, training, support and advice for the multidisciplinary team.
The focus of the multidisciplinary approach will be to support patients and carers to live well, prevent inappropriate hospital admission and meet their preferred priorities for care.
Part time (30 hours and above) would be considered.
Main duties of the job
An exciting opportunity has now arisen within South Manchester Macmillan Palliative Support Service for an Associate Palliative Care Nurse.
The post holder will work as part of the Macmillan Specialist multi-disciplinary team comprising of Medics, Dietician, Occupational Therapist, Physiotherapist, Nurses and Speech and Language Therapist. The team aims to provide effective support palliative care and/or rehabilitation to patients with a life limiting condition requiring palliative intervention.
The post holder will provide supportive palliative care to enhance the quality of life for patients / carers with any life limiting illness requiring palliative care, including complex physical, psychological and emotional needs.
Recent investment in palliative care has enabled the service todevelop substantially for the city of Manchester; thisis anexciting time to join our innovative and expanding team.
