Nurse Case Manager Band 6:
Manchester Case Management (MCM) is an exciting new service being developed in Manchester, building on the successful High Impact Primary Care (HIPC) pilots and Active Case Management services.
We are looking for person centred experienced nurses to join our friendly supportive team. You will experience a wide range of patients journeys and work closely with your colleagues to promote health education, well-being and plan care to improve patients lives and health across Central Manchester
MCM is a dedicated multi-disciplinary service pro-actively supporting people at high risk of hospital attendance and admission with a range of complex physical health, mental health and social needs. Working with the person and their family / carer, the MCM service focuses on what is important to each person in terms of improving their health and well-being. The multi-skilled team share the care of the person, and provide intensive and flexible support through clinic appointments, home visits and phone based consultations.
The MCM team includes GPs, Nurses, Social Workers, Pharmacists and Support Workers. GPs clinically lead the service which works with the wider Integrated Neighbourhood Team (INT) to draw in local community assets and services as part of the offer to MCM clients.
Main duties of the job
Working across the Central Manchester Locality the Case Manager will undertake the care of a defined caseload of patients working closely with and based within one of our four integrated neighbourhood teams .You will have the relevant experience and skills of a case manager to undertake this role.
The role requires the post holder to travel between the 4 neighbourhood sites and to patients homes.
