Job summary

We are looking for Band6 Occupational Therapist tojoin our Pathway 3 Therapy Team which is a dynamic, thriving and successful team within Manchester.

As a Band 6 Occupational Therapist, it is an exciting time to be part of our newly established and presently evolving team that is providing assessment to patients who are discharged from hospital as a short term measure with the aim of completing all assessments and making decisions about long term care needs.

Main duties of the job

The Pathway 3 Therapy Team currently consists of Occupational Therapists, Physiotherapists and a Rehab Assistant. As a team we work very closely with Pathway 3 Co-coordinators, Social Workers, Nurse Clinicians and other specialist community services. As a team we provide appropriate assessments to patients in a more suitable environment with a view to them potentially being able to return home living as independently as possible with the necessary support.

As well as bringing your clinical skills to the team, you will be a champion of Occupational Therapy and integrated community care. You will need to be a strong team player, an excellent communicator, adaptable, committed, well organized and work well under pressure. This position requires you to be a car driver and be able to travel around the three localities of Manchester to see patients in Discharge to Assess beds situated in Care Homes. Youmust be motivated, enthusiastic and have proven two years post graduate experience of working in a multi-disciplinary team.
