Job summary

We are Trafford Local Care Organisation - an exciting, new public sector organisation bringing together NHS community Health Services and Adult Social Care Services in Trafford. We are driven by the aim of improving the health of local people in Trafford by working across organisational boundaries.

You will be joining a team of experienced, friendly and supportive Speech and Language Therapists, with a strong professional structure in place.

We have regular team meetings and promote a climate of collaboration and discussion around clinical and professional matters.

The caseload is varied and patients can present with multi- factorial difficulties. Our team provides a service in the patient's place of residence and in out- patient clinic rooms.

You will take responsibility for assessing and managing patients who present with a range of swallowing and /or communication difficulties. We have regular access to SaLT-led Videofluoroscopy clinics to aid dysphagia management and decision-making.

This is an exciting time to part of community services in Trafford. Opportunities for learning and development and joint working across community AHP teams in the Greater Manchester footprint is supported and encouraged.

Main duties of the job

To engage in clinical triage of new referrals. To interpret and undertake advanced clinical reasoning to analyse detailed/complex clinical and non-clinical information provided. To make a justifiable assessment, contribute to the diagnosis and prognosis of a patient and to recommend the best course of SALT intervention.

To carry a caseload appropriate to role in line with banding and professional expertise.

To utilise specific SALT skills as required facilitating and optimising the treatment of patients.

To be accountable for own professional actions guided by registered body and code of professional conduct and other broad policies and guidelines.

To communicate sensitively and effectively with patients and their carers to ensure understanding of their condition and gain informed consent.

To utilise experience and clinical knowledge to enable effective communication when dealing with complex/difficult patient situations where there may be conflict and a lack of acceptance of a diagnosis, care programme and/or home situation.
