Job summary
The Community Stroke and Neuro-Rehabilitation Team is a well established multidisciplinary team with extensive skills and knowledge. It comprises of physiotherapists, occupational therapists, speech and language therapists, neuropsychologists, specialist nurses (stroke,neuro and parkinson's disease), dietitians, therapy technicians, support workers, and an OT apprentice and provides specialist rehabilitation for adults with a stroke, acquired brain injury and progressive neurological conditions. Conditions include MS, MND, Parkinson's disease and TBI. It is currently a very exciting time as we are expanding our service to provide a stroke and neuro rehabilitation service in line with the GMNISDN service specification.The team is based inSale and provides specialist rehabilitation programmes in patient's homes, clinics and outpatient facilities across the borough of Trafford.This post offers an exciting opportunity for a Speech and Language Therapist to consolidate their neurological rehabilitation skills within a well-established MDT which offers community stroke and neurological rehabilitation pathways. We provide services in client's homes and other community settings.
Main duties of the job
- To carry out the initial assessment of patients referred to the Community Stroke and Neuro- rehabilitation team (CNRT) team.
- To be responsible for providing subsequent specialist assessments for patients within the caseload to assist in the determination of the medical/clinical diagnosis and formulation of treatment plans, utilising goals agreed with the patient.
- To plan, deliver, evaluate, and modify specialist programmes of care for patients with complex medical conditions /social needs, providing advice to patients and carers in conjunction with the multi-disciplinary team and other agencies involved.
- To be actively involved in the use of research and audit to ensure that treatment is evidenced based and informs service evaluation and quality improvement initiatives.
- To support the Service Manager to ensure that support staff are clear about their responsibilities and accountability and that they have received appropriate training and supervision and are competent to carry out any duties that are delegated to them.
- To support the development of a culture of accountability and governance at all levels
- To maintain and develop close working links with other Speech and language therapists in the team.
