Job summary
An exciting new opportunity has arisen for one full time Band 7 Community Diabetes Specialist Nurse, 2 years fixed term to join us at Trafford Local care Organisation, the community arm of Manchester Foundation Trust. The post will involve working alongside GP practices, district nursing teams and other community-based teams across Trafford to offer support, advice, and education.
We also work closely with consultants, outpatient diabetes services and provide specialist support and advice to adults living with diabetes.
We require a motivated, dynamic nurse to join a friendly Community Nursing Teams. Effective communication skills are essential and being able to work both within a team environment and independently; organising and prioritising individual workload is crucial to this role. Experience in managing diabetes care is essential as is a willingness to learn new skills appropriate to the post.
The post will be suitable for nurses working within a diabetes specialist area who may want to expand their skills,
The post holder must be able to demonstrate excellent clinical, communication and interpersonal skills to maintain the excellent standards of diabetes care our patients expect.
Main duties of the job
The post holder will exercise higher levels of judgement, discretion and decision making in clinical care demonstrated through the Trust competency framework. S/he will provide specialist expertise and advice demonstrating a sound understanding of the issues related to the identification, assessment, treatment, and management of the client group. S/he will monitor and improve standards of care through, supervision of practice, clinical audit, evidence-based practice, teaching and supporting professional colleagues and the provision of skilled professional leadership. S/he will contribute to clinical governance within the service by undertaking audit and research as part of the MDT within the specialist field. Be responsible for acting as a lead source of clinical leadership through a highly developed level of competence, expertise, and advanced specialist knowledge within primary care settings.
