Job summary
We are looking for aNursing Associate/Associate Practitioner!
You will play a key role in supporting people living with diabetes to manage their condition and achieve their health goals. Working within agreed competencies and under the guidance of registered practitioners, you will contribute to the assessment, planning, delivery, and evaluation of care.
The role may involve visiting patients in their own homes or care homes and includes periods of lone working, requiring confidence, initiative, and excellent communication skills.

Main duties of the job
- To work within the multi-disciplinary Portsmouth Community Diabetes Service delivering community-based services to a GP Practice population, undertaking nursing tasks within agreed parameters.
- The job may include home/care home visiting and lone working without direct supervision.
- The role will focus on supporting patients living with type 2 diabetes to optimise their health and wellbeing.
- To offer support and education to older people, people with learning disabilities and mental health who are living with type 2 diabetes.
- The role will include educating, advising and supporting other health care professionals in managing patients with type 2 diabetes, including practice nurses, community nurses, GP's etc.
- The role will include providing education to patients and their families/carers regarding healthy eating, diet and lifestyle self-administration of medication and glucose level monitoring.
- To understand technology, apply sensors, monitor glucose levels, to download and interpret the data produced.
