You will be confident in taking on new responsibilities, with appropriate training, manage a varied case load and work holistically within a team. The service covers 7 days per week 8am to 8pm.

We are seeking full time applicants, however part time hours will also be considered. You will need to be computer literate although additional training can be provided. A valid driving license and access to a vehicle for work purposes is essential for this role. Although we can offer a lease car scheme to help.
Main duties of the job
To provide a high standard of clinical care, encompassing a holistic approach under the direction of a trained nurse, in patient's homes.
Working within the a defined community nursing caseload for a group of GP Practices as part of the Integrated Community Care Team.
The post holder will have responsibility for maintaining safe practice by following relevant Trust policies according to clinical setting, holistic nursing assessments and care plans are recorded in line with Trust and NMC record keeping standards and clinical care at all times meets acceptable standards of patient safety and quality outcomes.
The post holder will be required to work with a high degree of autonomy and professional accountability. A primary aim of the role is to prevent unnecessary admission of patients to an acute hospital and support timely discharge into the community. The post holder will also have a responsibility for health protection and promotion programmes that improve and reduce inequalities.
