Job summary
Southampton and Southwest division are seeking a geriatrician with an interest in community work to join our team. The role will be within a dynamic and innovative multidisciplinary team to offer both proactive and reactive care to patients within our geography.
Main duties of the job
The applicant will be expected to provide community based care and work within and strong multidisciplinary team. Strong communication, clinical and judgment skills are expected, to contribute to these services and also develop his/her interests.
Job planProvide two sessions to support frailty hospital at home & Urgent community response MDT and One team MDT: 2 PAProvide 2 sessions offering comprehensive geriatric assessment to patients living in their own home: 2PAProvide one clinic per week focussed on frail patients: 1PAA session for clinical administration: 1PAEducational and/or clinical Supervision for 4 month rotational FY2 post in community geriatrics and SpR: 0.5 SPASPA for appraisal and revalidation: 1.5 SPA
