Job summary
To meet the needs of patients and their families to achieve improved health outcomes and promote health and wellbeing while demonstrating care, compassion, competence, communication, courage, and commitment. Care is based on comprehensive assessment, ensures continuity and is patient/client centred, culturally sensitive and evidence based.
To develop and maintain a culture of service improvement which promotes quality and safety and striving to do better. This is in line with the requirements set out in the NMC code which require all registered nurses to Prioritise people; Practice effectively; Preserve safety; Promote Professionalism and Trust.
Community Health Nurses work with the 0-19 service, supporting the Children and Families team with the Healthy Child Programme and short- and medium-term interventions around behaviour and parenting.
To be an active member of the 0-19 Public Health Nursing Team supporting delivery of the Healthy Child Programme core contacts and delivery of the 6 high impact areas which are part of the Healthy Child Programme.
To implement and evaluate packages of care at Universal and Targeted levels of the Healthy Child Programme working closely with Health Visitors and Nursery Nurses.
To work in partnership with other agencies and families and contribute to innovative ways of working with children and families.
Main duties of the job
*To participate in the delivery of core contacts of the Healthy Child Programme to children who are part of the Universal and Targeted caseloads both in the home and in community settings.
* To undertake short- and medium-term interventions with Targeted families who have been assessed as needing help with healthy weight activities, child development and/or parenting interventions. This may be by home visits or groups or interventions in an education or local authority setting.
* Through supervision with line manager in 1 to 1s and within supervision groups the CHN will demonstrate competency in clinical decision making to assess and develop appropriate plans of care under the Universal and Targeted levels of the healthy child programme.
* To communicate effectively, providing and receiving sensitive and complex or contentious information where advanced inter-personal skills are required.
