Job summary

An exciting opportunity for a motivated Physiotherapist to join a dynamic multidisciplinary community rehabilitation service in Southampton. The Community Independence Service is integrated with Community Nursing and forms the West Locality based in Oakley Road, delivering programmes of rehabilitation working in partnership with other social and healthcare providers.

The role will allow individuals to gain experience in falls assessment, education and prevention, complex community rehabilitation and goal focused rehab planning.

Additionally, the role will involve provision of equipment and minor adaptations to support independence, providing advice on health and wellbeing and signposting to other community services. This will be supported by therapy staff, an advanced clinical practitioner and geriatricians.

The team promotes inter professional working which supports the single assessment process for patients. There is a robust supervision and appraisal framework within the team and opportunities for training and development. You will need effective interpersonal skills and to demonstrate skills to support a flexible and positive approach to problem solving.

Main duties of the job

Work as part of the multi-professional Locality Team delivering programmes of rehabilitation within a community setting in close collaboration with other relevant services/agencies e.g. other community and inpatient teams, social care, primary and secondary care.

Manage a caseload with supervision as required/be responsible for a defined area of service, undertaking comprehensive assessment, planning and implementation of comprehensive programmes of care for patients who require intensive support / rehabilitation guided by more senior clinicians where required.

Participate in and maintain own clinical professional development (CPD) by keeping abreast of new developments and incorporating into practice under guidance from supervisor and providing supervision and assessment rehabilitation assistants and students on placement.

Work collaboratively with partners in health and social care, to establish a whole systems approach to managing patients with complex care needs, maintaining a community pull on patients being admitted to secondary care, to inform decisions about acute interventions and to lead in the discharge planning.
