Job summary
Are you an experienced Nurse, Social Worker or Allied Health Professional who is passionate about the flow of the hospital and would be interested in a role supporting the discharge of patients from the Acute Hospital to home or an appropriate community setting? We are seeking a Trusted Assessor Team Lead.
This person will be responsible for developing and overseeing the quality of Trusted assessments in line with the new Hospital Discharge Support Guidance. To advocate for home first principles and follow the discharge to assess processes to deliver flow out of the acute setting.
They will be registered and have relevant previous UK NHS experience, have comprehensive post-graduate experience and experience developing and supervising junior team members.
Providing professional and clinical support and line management to the Transfer of Care Hub Team - ToCH - reporting and deputising for the Service lead.
Providing clinical expertise, assessment, advice and information, utilising clinical assessment and care pathways to ensure the most appropriate care for the patient via senior decision making.
Main duties of the job
Taking comprehensive history, undertaking overview assessments and providing telephone advice to patients and staff, facilitating onward referral to other professions.
Providing autonomous advice.
Liaising and communicating clinical and social care information verbally and via technical links to other professionals.
Provide leadership to the multi-disciplinary team in caring for patients.
Job StatementTo assess patients in the Acute hospital and to support flow from the Acute beds to the patients homes with appropriate care or community bedded settings.
To communicate work with all areas of ToCH and the MDT to achieve the best outcome for patients.
All actions are in line with NMC or HCPC codes and other statutory and organisational standards.
Educate and support staff. Identify staff development needs through the appraisal process.
As a senior member of the team, act as a role model to all members of staff, students and visitors.
Support the ToCH team to manage & monitor clinical standards in line with the Trust policies through audit process.
To provide information, including data, performance and notification of incidents and accidents as required.
Initiate the investigation of concerns related to poor clinical practice or staff management.
Escalate all concerns when there is a concern over the care being delivered by an individual to the appropriate lead.
