Job summary
We are looking for an experienced and compassionate Registered Nurse to join our friendly Community Nursing Team, based in Fareham. This role is a fixed term contract of 12 months maternity cover.
In this role you will work alongside patients and their families to achieve improved health outcomes and promote health and wellbeing while demonstrating care, compassion, competence, communication, courage and commitment. Care is based on comprehensive assessment, ensures continuity and is patient-centred, culturally sensitive and evidence-based and which promotes independence.
As a Community Nurse you will be responsible for completing patient visits each day, influencing clinical decisions and monitoring quality of patient care provided through utilising and maintaining expert clinical knowledge relevant to your own field of practice, underpinned by theory and expertise.

Main duties of the job
Act at all times with compassion, use effective communication and have courage when needed
Participate in the holistic assessment, implementation and evaluation of programmes of care and therapy for patients in the home, residential and nursing homes, clinics, community centres and community hospitals
Make appropriate referral - including safeguarding risks - to members of the Community Care Team or external organisations
Provide safe medicines management
Inform the Integrated Clinical (or Senior) Lead of any concerns with patients, relatives, visitors, or staff
Communicate effectively with colleagues, patients and carers
Promote person-centred practice
Ensure that patients with palliative care needs (and their families) have those needs met
Recognise own limitations in the provision of clinical care and urgency of patients' needs
Actively support other teams in the department when needed so that those teams can deliver high standards of care
Develop own professional skills and ensure these are shared with the team and wider Division
Use reflective practice to analyse incidents and events and to develop high quality care
Ensure practice follows evidence-based practice and national guidelines are followed
Actively participate in the HIOWH developments related to chronic disease management and targeted care.
Initiate policy and practice changes as a result of incidents, audits and complaints.
