Job summary
An exciting opportunity has risen for a full time experienced Registered Mental Health Band 6 nurse to join our community mental health team for older adults in the Totton and Hythe area .
Our experienced MDT are innovative and forwarding thinking offering opportunities for the successful candidate to work across Memory assessment service/CMHT and In reach pathways to deliver high quality person centred care to our client group and their carers and families.

Main duties of the job
To act as care co-ordinator to an agreed caseload of patients with severe and enduring mental illness. To participate within the multi-disciplinary professional team in the relevant care group to deliver safe patient care. To be an active participant in the multi-disciplinary team, formulating care plans and utilising appropriate assessments. To work in partnership with families and carers to ensure the patient's holistic health needs are met.
Understand evidence based therapeutic interventions relevant to the client group to include education, clinical care, wellness recovery action plans and discharge planning.
To participate in regular clinical, and managerial supervision.
To supervise junior staff.
