Job summary
Join a new innovative community health team delivering proactive, person-centred care across Southampton neighbourhoods. As a Neighbourhood Care Practitioner, you will work at the intersection of primary care, social support, and community services to prevent hospital admissions, support independent living, and improve wellbeing for people in their homes and local settings. This is a hands-on, autonomous role with strong clinical collaboration and opportunities to shape service design.
Posts available: Three full-time posts, one in each of the three localities across Southampton with cross cover when applicable.
Contract: Fixed 12-month contract with the possibility of extension following evaluation.
Hours are full-time, flexible working patterns considered.

Main duties of the job
The Neighbourhood Care Clinician role will work with Primary Care Network (PCNs) practices to proactively management patients living with frailty and other long-term conditions. In conjunction with the wider health, social and voluntary care workforce this role will lead and facilitate patient centred co-ordinated case management approach across primary, community and secondary care for people who are most vulnerable to and at high risk of admission to hospital.
Working autonomously to a high standard of clinical care, the clinician will be responsible for liaising with the PCNs to identify patients living with frailty who would benefit from an intervention to either prevent further deterioration of their frailty or other long-term conditions and to prevent hospital admission.
Using the medical model to assess, diagnose and prescribe for acute medical conditions, the clinician will use a holistic approach to consider methods and interventions to prevent recurrence of problems and complete referrals to appropriate members of the wider multidisciplinary team.
Following clinical assessment using comprehensive geriatric assessment either in the patients home or clinic environment, multi-disciplinary patient pathways will be designed to support patient choice, improve quality of life, promote self-management and assure early intervention through proactive provision of care in or close to the patients own home as possible.
