Community Learning Disabilities Nurse

Job summary

We have an exciting opportunity for a fixed term, part time position, as an experienced RNMH/RNLD nurse to join us within the multidisciplinary Community Learning Disability Team on the Isle of Wight. The post holder will be required to have RNMH/RNLD qualification and experience of working with people with learning disabilities who experience complex physical health and mental health needs.

The applicant will need to demonstrate a genuine interest and commitment in working with people with a learning disability, their carers and as part of a multidisciplinary team. The applicant will also need to be able to work in a very busy environment, work as a lone practitioner and be able to prioritise workload and be flexible to the ever-changing needs of the individual and service.

A car driver is essential for this post.

Main duties of the job

To provide high quality care and treatment in conjunction with other professionals and as required work in partnership with other agencies to provide a seamless, flexible services which meet service user and carer need.

To actively promote the health and wellbeing of people with learning disabilities through a range of specialist clinical and education interventions.

To provide specialist advice on a range of health issues, and work to rescue the health inequalities that people with learning disabilities face.

To be professionally accountable for a caseload of clients who may or may not be in receipt of Continuing Health Care (CHC), or S117 funding of the Mental Health Act.

Contribute to and assist with the care coordination of individuals with LD who are placed out of area in specialist hospital or residential settings.

To supervise and appraise designated staff.

Support and contribute to the development of the learning disability service in accordance with the vision and plan outlined in the Isle of Wight Learning Disability Strategy.
