Job summary
*To work alongside primary care to support and deliver therapeutic interventions to patients in primary care with mild to moderate mental health needs.
*To liaise effectively between primary and secondary care so that referrals can move seamlessly between services, prioritising the patients journey.
*To be professionally accountable and responsible for patient care.
*To triage and undertake initial assessment of presenting need and risk, signposting to appropriate services, or providing time-limited psycho-social therapeutic interventions directly, in GP surgeries, patient's homes and other primary care settings.
*Provide clinical advice to GPs such as specialist opinion, this will contribute to the safe and effective day to day operation of the PCN and the efficient and timely management of clinical enquiries and decision-making.
*To represent patient need at multi agency meetings.
*To jointly work with other agencies to meet patient need.
*To maintain clinical knowledge and skills by attending training and applying this in practice to support the development of the team.
Main duties of the job
*To participate in MDT meetings.
*Communicating service user needs and be able to assist in prioritising the allocation of workload.
*To assess risk, develop risk management plans, review and adjust in line with the service user's needs.
*To work in collaboration with patients as experts in their own care, promoting the principles of recovery, choice and personalisation, which value and empower the service user.
*To assess, coordinate and broker the needs of services users in primary care and other relevant agencies in partnership with the service user.
*To accurately record all contacts with service users in the appropriate care system in line with the relevant record keeping policy.
*To ensure effective communication with service users, relatives and carers, visitors, staff and others.
*To ensure that all communication takes place in a manner that is consistent with legislation, policies and procedures.
*To ensure that consent to intervention or assessment is sought in a manner that is meaningful to the service user and complies with legislation.
*To respond to questions or concerns from service users and carers in a timely and professional manner.
*To present a positive image of the team, service and organisation.
*To assess and plan care in a manner that is consistent with evidence-based practice, policies and procedures and the management of risk.
