Main duties of the job

We currently have the below opportunities available:

A full time (37.5 hours per week) role covering Eastleigh and Romsey based at Newton House and operates Monday-Friday 9am-5pm. This is an Older Person's Community Mental Health Team supporting older people with mental health needs, including those with Dementia.

A part time (30 hours per week) role in the West New Forest area (Ringwood and Fordingbridge) which operates 0800 to 2000, 7 days a week.

A part time (22.5 hrs per week) role covering the East based out of Bitterne Health centre operating 0830 to 2000, 7 days a week.

Full and part time roles in our central location based from Thomas Lewis House operating 0830 to 2000, 7 days a week.

Part time(22.5 hrs per week) role covering Chandlers Ford which operates 0800 to 2000, 7 days a week.

Please note that other than Older person mental health team the roles require weekends and bank holidays, with an expectation of working 1 in 4 weekends.

To work within the multi-disciplinary Integrated Care Team that delivers community based services to a GP Practice population, undertaking nursing tasks within agreed parameters, set out by the registered practitioners.

The job will include home visiting and lone working without direct supervision. The scope of the role will focus on supporting admission avoidance, supported discharge and promotion of independence and assisting in the effective management of patients with long term conditions and older people with mental health problems.
