We are currently recruiting across our community nursing teams at Hawthorn Lodge (22.5 hours), Bittern Health Centre, Oakley Road, and within our Urgent Response Service Team based at Oakley Road.
Our services support patients complex care and treatment to adults living in the area who have long-term or complex health needs in their own homes. You will work in a multi-disciplinary way alongside a number of specialist services, including those in the community, social and voluntary services.
We are seeking compassionate, proactive nurses who are eager to develop their clinical and leadership skills in a community setting. Main duties of the job
As a Community Staff Nurse, the post holder plays a vital role in delivering high-quality care through clinical expertise, leadership, and professional integrity. Working across diverse community settings, they will engage in clinical supervision, attend developmental training, and contribute to service improvement. Adherence to the NMC Code of Professional Conduct is essential at all times.
The post holder will promote effective communication and to manage a defined community case load. They will support complaint resolution and uphold professional standards. The primary aim of the role is to prevent unnecessary admission of patients to an acute hospital and support timely discharge into the community.
Clinically, the post holder will assess referrals, plan and evaluate care for individuals with complex health needs. They will also have a responsibility for re-enablement rehabilitation, health protection and promotion programmes that improve and reduce inequalities. The post holder will also uphold safety policies during home visits, maintaining vigilance to safeguard patients and staff.

Bitterne Health Centre: Full Time, 7-day service
Oakley Road: Full Time, 7-day service
URS (Oakley Road): Full Time, 7-day service
