Job summary
We are recruiting for a Referral Coordinator to join our Single Point of Access (SPOA) team on the Isle of Wight.
You will conduct comprehensive patient assessments, provide expert advice, and direct patients to appropriate resources as a key member of the Community Single Point of Access (SPOA) team.
Your role will be to serve as a referral coordinator, executing referral reviews and telephone triage within a collaborative, inter-professional team.
Collaborate with healthcare professionals to deliver high-quality services to clients across a diverse range of community services.
Engage in an integrated service model, delivering multifaceted interventions. Please note that this role may necessitate weekend and bank holiday work on a rotational basis in the future.
Main duties of the job
Serve as the initial point of contact for service users, providing a welcoming and supportive environment.
Function as a personal guide and support for service users throughout their treatment journey.
Advocate for service users, ensuring they are on the correct treatment pathway.
Perform as a key operational communicator between staff and service users, facilitating clear and effective communication.
Promote the active participation of all clinicians and registered staff in delivering services to the service user.
Collaborate closely with Local Authority and other care services to ensure a seamless interface within local care provision.
Coordinate all referrals for community services across the localities, allocating patients to suitable staff members with a priority on Urgent Community Response (UCR) 2-hour patients.
Demonstrate a comprehensive understanding of the Musculoskeletal (MSK) guidelines and pathways to ensure patient onward referral criteria and processes are well understood and implemented.
