An opportunity has arisen for a motivated person to join the West Locality within Southampton as Multi-skilled Support Worker. The service provides rehabilitation, reablement and specialist falls intervention, community nursing input and a housebound phlebotomy service. This post will be based predominantly within the rehabilitation team but will provide support across all nursing and phlebotomy depending on need, skills and experience.

Previous experience of working within a care setting would be desirable. You will need great interpersonal skills and positive communication skills and be able to problem solve. We will provide support and training to help develop your knowledge and skills.

We have one full time position (37.5 hours per week) available. Service hours are 8.30am to 5.00pm Monday to Friday. For this post, you will need to hold a full UK driving licence and have access to the use of a car on a daily basis.

Main duties of the job

Provide flexible support across community nursing, phlebotomy and the community independence services depending on demand including:

Supporting the delivery of care to clients within their own home environment.

Ensure that patients receive safe, individualised and effective standards of care by following care plans and treatment interventions as delegated by the registered nurse or therapist.

Under delegation of the nurse/ therapist in charge, work competently and without direct supervision within an agreed framework

Work across within the Southampton and South West division of HIOWH
