We are looking for Registered Nurses and Midwives to join our Health Visiting team as a Community Health Nurse (CHN) in Southampton City supporting children, young people and families in their own homes and community settings. We welcome applications from all Registered Nurses, Adult, Child, Mental Health, Learning Disabilities, and Registered Midwives.

You will work with children aged 0-5 and their families, alongside experienced Health Visitors to deliver the Healthy Child Programme (DH 2021).The Healthy Child Programme offers every family an evidence-based programme of interventions including screening tests, immunisations, developmental reviews and information and guidance to support parenting and healthy choices.

This role will draw on your previous knowledge and skills to ensure the best outcomes are achieved and care is delivered to children and families in Southampton at the right time and in the right place by the right professional. Main duties of the job

Community Health Nurses work within the 0-16 Public Health Nursing Team, delivering the Healthy Child Programme through the 5 mandatory contacts and through short- and medium-term interventions which include infant feeding, behaviour and parenting.

To plan, implement and evaluate packages of care at Universal, Universal Targeted and Specialist levels of the Healthy Child Programme which is led by Health Visitors and Public Health Nurses.

To work in partnership with other agencies and families and contribute to innovative ways of working within children and families.

Work with Children and families in the 0-16 service but mainly 0-7 yrs.

The work will take place in the community either patients own home a dedicated sure start centre, health venue, or education venue also through digital platforms.

The role involves participating in the delivery of the healthy child programme. To undertake short- and medium-term interventions to support children and families with the healthy weight pathway, behaviour support, school readiness and the transition to school as well as parenting to improve outcomes for the children. To deliver evidence-based interventions on a 1 to 1. To deliver group interventions this may be face to face or via a digital platform e.g. introduction to solids. To develop innovative and creative ways of promoting public health messages. This will include helping to meet the needs of children with complex needs.
