Job summary
We have a fantastic opportunity for an enthusiastic and motivated individual with relevant experience in Mental Health to join the Southern Health Perinatal Service on a secondment or fixed term contract.
The Hampshire, Frimley and Isle of Wight Perinatal Mental Health Service is a well established and pioneering specialist service consisting of an expert multidisciplinary community team and a mother and baby inpatient unit. The service is also an early implementer site for Maternity Mental Health Services, broadening the remit and profile of specialist interventions available for women/birthing people/birthing people and ensuring an enhanced care pathway. The Perinatal Community Team provides a specialist community service for the assessment and treatment of women with severe mental health problems in pregnancy and up to 2 years postnatal.
We are looking for an experienced Mental Health Nurse, Occupational Therapist or Social Worker with an interest in the field of Perinatal Mental Health.
The available role is within our South East Hampshire locality team who are based at Fareham Community Hospital Car Driver is essential.
Main duties of the job
The role of the Perinatal community mental health nurse/practitioner will be to undertake comprehensive assessments, manage a complex caseload of service users, provide practical advice and support including risk management, psychological interventions and overseeing any safeguarding issues. Care coordination and care planning within the Multidisciplinary Team is a crucial part of this job to ensure patient led, collaborative, high quality care is being delivered. The successful candidate will be working closely with their locality colleagues as well as the wider perinatal team including parent-infant practitioners, psychology and medical staff to ensure our service users are given the best care and experience, as well as working with outside organisations to ensure robust joint working where needed.
