Job summary
The primary purpose of a Band 5 Nurse on an Older Persons Ward is to provide direct, compassionate, and effective nursing care to elderly patients, many of whom may have multiple medical conditions, dementia, mobility issues. The nurse is integral in supporting the patient's dignity, safety, and comfort while ensuring optimal care in line with evidence based practices.
Main duties of the job
Band 5 nurses deliver high-quality, person-centred care to elderly patients across clinical and community settings. Responsibilities include comprehensive assessments, developing and reviewing individual care plans, administering medications, and performing essential nursing interventions such as wound care, catheter care, and feeding support. They monitor patients' physical and mental health, identifying signs of deterioration and reporting promptly. Specialised dementia care is provided using non-pharmacological approaches in calming environments. Nurses liaise effectively with families and multidisciplinary teams to ensure holistic care and emotional support. They promote health through preventative strategies for pressure ulcers, falls, malnutrition, and dehydration. Accurate record-keeping ensures compliance and continuity. Safeguarding and risk management are key duties, alongside supervising junior staff and participating in ongoing professional development.
