Job summary

Are you interested in becoming part of an innovative community Older Persons Mental Health (OPMH) Service, which focuses on all areas of mental health, irrespective of functional, organic or complex illness on the Isle of Wight?

We are aligning our focused OPMH pathways with all age Community Mental Health Services and are looking for passionate, highly motivated individuals to work as part of the Older Persons Mental Health Service. The post holders will provide high quality mental health nursing care to a caseload of adults over the age of 65 years with complex functional and organic mental health problems, to include dementia.

You will be responsible for ensuring that the highest quality care is carried out in accordance with policies and procedures Hampshire and Isle of Wight NHS Foundation Trust.

You will need be a Registered Mental Health Nurse (RMN) and have experience of working with people with serious and enduring mental health needs. The ability to work in partnership with carers/family members and services in the community is essential.

Main duties of the job

The role will primarily be delivered in a community setting, including patients' homes, residential and nursing care homes, nurse led clinics. The nurse will work as part of a multi-disciplinary team (MDT) and collaborate with GPs, social care, pharmacy teams, voluntary agencies, and acute and crisis services to ensure safe and effective care planning.
