In this vital role, you will help deliver safe, high-quality, and effective services that prevent unnecessary hospital admissions, support early discharge, and manage patients with complex long-term conditions or acute health episodes. You will also provide personalised end-of-life care for patients and their carers.

You will be expected to work autonomously and with professional accountability, managing a defined community nursing caseload and stepping up in the absence of the Community Sister or Clinical Manager. Your practice will align with Nursing and Midwifery Council standards to ensure excellent patient outcomes.

You will be based at one of the following locations; please indicate which location is your preference; Royal South Hants Hospital, Western Community Hospital, Bitterne Health Centre, Western Community Hospital - Broadcast, New Kings Court and Hawthorne Lodge.

Main duties of the job

This is a community-based Registered Nurse role with vacancies across multiple locations across Hampshire and the Isle of Wight, delivering high-quality nursing care in patients' homes and other community settings. This includes first-contact care such as acute assessment, nursing diagnosis, treatment and referral. You will also provide palliative care, focusing on symptom management, assessment of acute symptom onset, and compassionate support for both patients and their families or carers. In addition, the role encompasses continuing care, including intermediate care, rehabilitation, and the management of chronic conditions. A key element of the position also includes contributing to public health by supporting health protection and promotion programmes aimed at improving health outcomes and reducing inequalities across the community.

  • Ability to act on own initiative in crisis situations and take responsibility for own caseload of clients.
  • Is able to work flexibly in a demanding and challenging environment.
  • Experience of conducting a basic audit.
