Pulmonary Rehabilitation Practitioner

Job summary

The Southampton COPD team works together with University Hospital Southampton to provide an integrated approach to COPD management for the population of Southampton. We provide a Pulmonary Rehabilitation Pathway and Community COPD clinics for people with complex COPD.

The Pulmonary Rehabilitation team currently consists of Physiotherapists, Respiratory Nurses, Exercise Instructor and Therapy Assistants.

The service is predominantly based at Bitterne Health Centre, but also run Pulmonary Rehabilitation exercise classes and community COPD clinics at Royal South Hants Hospital and Adelaide Health Centre.

We are looking for an enthusiastic practitioner who will actively promote the COPD pathway and primarily support the delivery of pulmonary rehabilitation in the community. They will independently manage and deliver evidence based PR exercise programmes to patients in different community settings, including digital programmes. The post holder will also support community respiratory clinics across the city supporting consultants and specialist practitioners in delivering complex COPD care.

The successful candidate will work as an active member of the team in supporting and leading patients during their time on our programme. They will have in depth knowledge of exercise and its prescription, being able to lead and progress patients through their programme. With this knowledge they will also be a resource for training of other team members and the development of the service.

Main duties of the job

*Complete referral triages, assessments and review of patients for Pulmonary Rehabilitation (PR).

*Implement the delivery of PR programmes for groups in face to face and virtual settings.

*Independently manage allocated PR venue, including caseload, logistics, equipment and staffing.

*Support patients in line with national and local guidelines, monitoring progress, modify and adapt treatment programmes as required using SMART goal setting.

*Work independently, within level of competency, to undertake assessments and interventions, recognising their own limits and request supervision of senior clinicians to ensure the delivery of safe care.

*Be aware and take appropriate action of clinical deterioration of patients, emergencies, internal and major incidents which affect service continuity.

*Provide direction and guidance for patients and carers on treatment options; physical, emotional and social.

*Have the ability to support patients with mobility problems, in an enabling way, including the use of wheelchairs or walking aids.

*Make person centred, evidence-based judgements in partnership with others involved in the care process to ensure compassion, dignity and high-quality care.

*Respond to patient, relative and carer's concerns and take action as required and escalate appropriately.

*Take a lead in working with 3rd party organisations to facilitate pathways to support long term behaviour change.

* Be active in organising and delivery of national awareness events e.g. World COPD day.

Essential

  • BA/BSc in Sport Science/Registered Nursing/Registered AHP or equivalent
  • Good all-round basic education to GCSE or equivalent

Desirable

  • Evidence of post graduate study/specialist training in respiratory/COPD
  • Level 4 Exercise Training for Chronic Respiratory Disease or Cardiac Specialist Exercise Instructor Training (will need completing if appointed)

Experience

Essential

  • Knowledge of physiological and psychosocial problems associated with chronic respiratory disease

Desirable

  • Previous experience working with people with co-morbidities in an exercise setting
  • Previous experience as an Exercise Technician or Instructor or within a respiratory specialism area
  • Experience of working in a community healthcare setting
