Job summary

An opportunity has arisen for a Registered Nurse, within our Complex Health Team, within Portsmouth's Integrated Learning Disability Service (ILDS) hosted by Portsmouth City Council.

As a nurse within the Complex Health Team, you will work as a member of the multi-disciplinary specialist health care team, within the integrated learning disability service. Your role will be focused on people with slighter higher and more complex healthcare needs, in receipt of CHC funding.

You will be supporting people to get the best care that they can to live their lives to the full.

You will be supported by an experienced team, with regular supervision and hybrid working options available. We can offer you high-quality professional support and a strong commitment to personal development. As a member of the team, you would receive an induction program linked to your particular needs, regular supervision and access to internal and interagency training. There is peer support, regular team supervision meetings, group and individual clinical supervision and regular line management meetings.

Your role will be varied, working with service users, family, and carers that you will establish and maintain positive professional relationships with.

Main duties of the job

The successful applicant will provide person centred assessments and interventions for adults with Learning Disabilities who have associated complex health needs in partnership with families, other agencies and carers. As an enthusiastic and well-motivated professional you will possess strong interpersonal, communication and literacy skills, demonstrating an ability to work with a range of professionals, agencies and carers, and manage a clinical caseload involving assessment, clinical nursing intervention and co-ordination of services.

Essential

  • *RNLD or RGN (with appropriate experience and knowledge) or equivalent essential - (other parts of the Nursing Register may be considered dependent upon relevant experience)
  • *Teaching & assessing
  • *Full clean driving licence

Desirable

  • *Degree in Nursing
  • *Other relevant post graduation or specialist training

Experience

Essential

  • *Experience of working with People with a learning disability, within a variety of settings
  • *Experienced in developing care plans
  • *Experienced in developing and delivering care and clinical interventions to patients
  • *Experience of MDT working
  • *Experience of teaching, training and / or supervision
  • *Knowledge of the Continuing Health Care process or a willingness to train.
  • *Evidence of recent CPD
  • *Evidence of developing policy, guidelines contributing to management of resources
  • *Excellent communication skills, written and verbal
  • *Clinically competent at Band 5 level
