We are seeking an individual to support the next stages in the development of the newly established CAMHS transitions service. This role will be a pilot, and it will run for 12 months initially, but the hope is that this role will become permanent, so we can continue to support young people from our CAMHS inpatient settings to the next stage in their recovery.

We adopt a holistic approach, not only focusing on mental health but also addressing the physical, social, educational, and motivational needs of the young people we work with. By employing the "social prescribing" model, we help young people engage with life through activities that promote community connections, sports, education, employment, housing, and more. Your role will empower young people to build confidence, improve wellbeing, and work towards their long-term health goals.

Main duties of the job

Join our dynamic team at Serenity Path Hospital and the wider community to make a real difference in the lives of young people and their families. As a Transitions practitioner, you will play a key role in supporting young people and their families/carers throughout their inpatient admission process and assist with discharge planning, offering continued support when they return to the community. This is an exciting opportunity to work within a unique and specialised CAMHS team that provides support both in the hospital and the community.

The purpose of this role is to work as part of the multi-disciplinary team at Serenity Path Hospital and Core CAMHS Service supporting young people by providing therapeutic interventions and support as they are discharged and move between services.

  • Degree level or equivalent experience in working/supporting children and young people.

Desirable

  • Degree/Diploma in Nursing, Social Work, Clinical Psychology, or Occupational Therapy.
  • Current professional registration with the relevant body (e.g., NMC, HCPC, BPS).

Experience

  • Job Specific Experience:
  • * Significant experience of working with young people, family's/carers.
  • * Experience of working with people.
  • * Experience in multi-agency working.
  • * Awareness of Mental Health Issues and knowledge of Mental Health Act.
  • Managerial/Supervisory Experience
  • * Experience of working autonomously and managing individual caseload to include lone working, but has a manager to refer to.
  • * Experience of supporting and working as part of a team.
