Job summary

This is an exciting, and innovative post to support the needs of the local population living with mild to moderate mental health difficulties. We are looking for a registered nurse, occupational therapist, or social worker to take a central role in the development of the mental health offer in one of our Primary Care Networks (PCN). Through the delivery of excellent, compassionate services to promote the recovery and well-being of patients, carers and families.

We support the primary care population of working age and older adults, linking with Talking Therapies Hampshire, No Wrong Door Inclusion, Community Mental Health Teams and Crisis Resolution Home Treatment Teams.

Main duties of the job

*To work with the GP practices in Abbeywell, Alma Road and North Baddesley surgeries to support and deliver therapeutic interventions to patients in primary care who do not meet the criteria for secondary mental health services and help deliver the transformation work around "No Wrong Door"

*To support the planning and coordination of service users' mental health care in primary care, liaising with teams and agencies as required. To work on signposting to appropriate agencies for ongoing support

*To liaise effectively between primary and secondary care so that referrals can move seamlessly between services, prioritising the patients journey

*To be able to deliver therapeutic interventions to service users whose immediate needs may need help in a primary care setting.

Qualifications

Essential

  • Qualified as a registered mental health nurse, social worker or occupational therapist
  • Mentorship course / evidence of CPD
  • Ability to fulfil travel requirements of the post
  • Clinical supervision training
  • More than 2 years' experience of working with clients with severe and enduring mental health needs

Desirable

  • Completion of in-depth safeguarding training
  • Additional qualifications in psychosocial interventions, substance misuse or brief interventions are desirable

Experience

  • *Excellent risk assessment and crisis/contingency planning skills
  • *Ability to triage referrals using clinical judgement skills
  • *Awareness of common acute and long-term conditions seen in general practice
  • *Evidence of effective team working and leadership skills
  • *Evidence of effective interpersonal and communication skills (active listening, building empathy) and ability to negotiate and constructively challenge views and practices of other practitioners
  • *Ability to organise self and others to achieve tasks
