As part of this initiative, we will be moving into the Chantry Centre Health Hub this summer and be co-located with other partner services to make patient journeys and transitions more seamless.

As this role is community based, applicants will need to hold a full UK driving licence (or equivalent) and have access to a suitable vehicle for work.

Main duties of the job

To be an active member of the multi-disciplinary team supporting the assessment, care planning and care delivery for our service users.

To act as care co-ordinator to an agreed caseload of service users with severe and enduring mental illness.

To undertake ongoing assessment, treatment and evaluation for service users known to the service

To support the delivery of therapeutic interventions either individually or within a group setting.

Support the transformation of the team so it delivers best care in line with contemporary evidence.
