Job summary
We have a rare and wonderful opportunity for a Health Care Assistant to join us here in Alton where we have a friendly team of HCP's working in the community to help patients who are unable to use primary care facilities due to frailty or disability be that temporary or long term. We are located centrally in a wide, rural area in the heart of the beautiful Hampshire countryside.
Community staff have a very diverse range of skills which create new and different patient experiences every day. Amongst many other services we offer Venepuncture, complex wound care including TNP and compression therapy, suture and clip removal, leg ulcer management, regular and non-regular injection treatments. The diverse care and skills we have, give us the unique opportunity of getting involved with not just patients but their families and lots of other professionals -- this can be within our 3 friendly surgeries, Boundaries, Chawton Park and Wilson practices or those broader services including therapy provision, social prescribing and adult social care.
All Training and supervision is provided until you feel confident that you can use your new found skills independently -- this can take as long as you need it too as we all have different learning paces and levels of confidence.

Main duties of the job
Catheter Care
Assist with the insertion, maintenance, and removal of urinary catheters as directed by nursing staff.
Provide care to the surrounding skin, ensuring cleanliness and comfort for the patient.
Educate patients and carers on catheter care, including hygiene practices and signs of infection.
Support patients with self-care practices related to catheter management where appropriate.
Venepuncture
Perform venepuncture ensuring accurate blood sample collection
Observe and record patient reactions during and after the procedure
Medicine Management
Assist patients with the administration of prescribed medications, including oral, topical, and subcutaneous as per the care plan.
Support patients in understanding their medication regimen and offer encouragement for adherence.
Record medication administration and ensure accurate documentation according to healthcare protocols.
Acute and Complex Wound Management
Assist with dressing, and assessment of acute and complex wounds
Educate patients and carers on signs of infection, and the importance of following prescribed treatment plans.
Support the management of pressure ulcers, or other complex wound conditions
Maintain accurate records of wound assessments, treatment plans, and progress.
Additional duties
Assisting in the delivery of personalized care plans
Communicating effectively with patients, families, and the multi-disciplinary team to ensure holistic care.
